These Registration Documents must be filled and submitted during Registration.

The Medical form is to be completed by a Medical Officer, for  students in Year 1 & 3 Only. The Sports Waiver is to be completed by Parents/Guardian for students in Year 1 Only. Conditions for Enrollment is to be completed by every student. The same three (3) documents are attached in your respective letters.
